DESCRIPTION: Made it thread safe by makeing the modify, create and delete

DESCRIPTION: All .h, .cpp and the wql, cql, compile *.l and *.y files along
with the .java files in providermanager2/jmpi updated to use the license
statement in doc/license.txt
Tested on windows and visual test of all diffs.

TITLE: Copyright update

DESCRIPTION: Update all .cpp and .h files for new license and
update the doc/license.txt file.  Note that there were also
a couple of files that had to be fixed because they had violated
the comments rules (ex. blank line at head of file or in the case of
xmlwriter.cpp a comment line //=========  which drove the strip
function nuts.  These were fixed.  This has been compiled and tested
on windows.

TITLE: SampleProvider breaks build on Linux

DESCRIPTION: Took a blind whack at fixing this while at home on a Windows
system by modelling the creation of the CIMObjectPath after existing bits of code.
With any luck, this *may* prevent some build breaks before I have a chance to
build on Linux first thing in the morning.

TITLE: TestClient fails processing enumerate instance operations

DESCRIPTION: Updated provider to always return object paths with the instances
it manages. This is need primarily because this is a sample provider and illustrates
"good form." Also, it allows the provider to work with the test suite when the
ObjectNormalizer is enabled or disabled.

TITLE: Sample providers should not be declared in Pegasus namespace

DESCRIPTION:

Removed PEGASUS_NAMESPACE_BEGIN and PEGASUS_NAMESPACE_END macros from
sample providers, added PEGASUS_USING_PEGASUS; so CIM symbols residing
in the Pegasus namespace will resolve.

HP-RK Update license header.

HP-RK Update sample providers to not use the Pair class template, which is not part of the Pegasus SDK.

HP-RK Use CIMObjectPath class instead of CIMReference.

HP-RK Updated the Instance, Association, and Class provider interfaces to use CIMPropertyList instead of Array<String> for the propertyList parameters.
