TITLE: Use of indices in IndicationOperationAggregate aren't thread safe

DESCRIPTION: Modified IndicationOperationAggregate to make the deleteRequest() and deleteResponse() methods private, and added doc++ to document limitations, responsibilities, and assumptions.
