DESCRIPTION: Update all .cpp and .h files for new license and
update the doc/license.txt file.  Note that there were also
a couple of files that had to be fixed because they had violated
the comments rules (ex. blank line at head of file or in the case of
xmlwriter.cpp a comment line //=========  which drove the strip
function nuts.  These were fixed.  This has been compiled and tested
on windows.

HP-RK String class rework from API review.  Added a StringRep class to encapsulate the String implementation.  This removes the String.h dependency on Array.h, which allows the Array template to explicitly throw exceptions (which depend on String, which used to depend on Array...).

HP-RK Removed the use of PEGASUS_TEMPLATE_SPECIALIZATION.

HP-RK Update license header.

HP-RK Reworked the Array template so that the implementation is hidden from clients and providers.  This was accomplished by using explicit instantiation of the array template for each array type that is supported in the client and provider APIs.  The array template implementation is still available for use within the Pegasus core, allowing implicit instantiation of additional array types used internally.

HP-RK Code cleanup.

HP-RK Remove unnecessary constructor.

HP-RK Reworked the Array template so that each array has its own ArrayRep copy rather than Arrays sharing ArrayReps and reference counting.

HP-RK Added a Mutex to the ArrayRep structure.  This Mutex gets locked in the inc() and dec() methods that manage the reference count.

HP-[RK] Apply array template signature fixes for Linux.

HP-RK Work around memory leak.  Fix InitializeRaw method to call constructors instead of destructors.  Switch the order of reserve() and _copyOnWriter() methods so that _copyOnWrite() is done first.
